As the children arrive a daily
observation of each child for
symptoms of illness and for signs
of unusual injury shall be made by
the teachers. In the case of
recognizable signs of
communicable diseases or other
evidence of ill health in a child.
parents or an emergency contact
will be notified immediately.
An ill child shall have a mat and
blanket and adult supervision. The
child will be isolated from the rest
of the class, but remain in the
classroom.
Written permission from the
parents to call the family physician
or Wauwatosa Fire Department-
Paramedic Squad in case of
accident or emergency shall be on
file. The parents shall be notified
as soon as possible in that event.
In case of a minor injury, the
teachers will treat with soap, water
and bandages, and will report the
injury to the parents at the
conclusion of the class. The injury
will be recorded in the school’s
medical log and a copy placed in
the child’s file.
In case of serious accident/injury,
the Wauwatosa Paramedics will be
called immediately, and the
parents, as soon as possible.
The teachers will follow standard
first aid procedures in case of
serious accident or injury.
A nutritious mid-session snack
will be served consisting of a
variety of foods and juices.

It is the responsibility of the parents to see that
their child has had a physical examination by a
licensed physician not more than one year prior to,
nor later than the day of admission to the school.
The medical form, signed and dated by the
licensed physician and the health history form
signed by the parent, shall be on file in the school.
A record of immunizations is also due by the first
day of school. Any allergies to foods or
medications need to be clearly stated on the
health history form. Medical documentation
required includes:
. . . An immunization history which states that the
child has been immunized against diphtheria,
pertussis, tetanus, poliomyelitis, measles
(rubeola), German measles (rubella), mumps,
hepatitis B, and Hib and Varicella (or chicken pox).
Notice of immunizations administered after the
form has been submitted to CCNS shall be given
to the school in written form stating type and date,
as soon as possible according to the
immunization schedule.
. . . If an immunization is not permitted by the
child’s physician, it shall be so stated, dated and
signed by the physician.
. . . If the child’s parents because of religious or
personal conviction do not permit an
immunization, it shall be so stated, dated and
signed by the parent.
. . . Each child shall have a subsequent physical
examination at least once every two (2) years by a
physician on file at the school.
REMEMBER: Medical and immunization
forms must be on file by THE FIRST DAY
OF SCHOOL.
