Once accepted, the following forms are to be
completed and on file at the school for each
child BY THE FIRST DAY OF CLASS.
Failure to bring in completed forms is cause
for suspension or dismissal:
1. Child Health Report (pdf)
2. Immunization Form (pdf) (only an
update needed for
returning students).
3. Health History (pdf)
The children’s files containing the forms listed
above are confidential and always available to the
parents upon request.
Curiosity Corner also has copies of all the center’s
policies, guidelines, and state daycare regulations,
which are available to the parents upon request.

To apply for enrollment, please submit a
$100 non-refundable deposit (to be used
toward tuition) AND the Application form
to:

Applications for enrollment are considered in the
order in which they are received. Eighteen (18)
children per class will be accepted for the school
year. Parents will receive a notification confirming
the school’s receiving the submitted application and
registration fee. However, if enrollment is filled, the
administrator will notify you immediately. Notification
of fall events will arrive by mail during the last half of
August.
Requests will be accepted for two (2) children (only),
to be placed in the same class. Both families must
make requests in writing, at the time of application.
CCNS reserves the right to make final decisions for
class placement based on what is best
educationally.
